What is an Accountable Care Organization (ACO)?
An ACO is a group of doctors and other healthcare providers who agree to work together with Medicare to give the best possible care. ACO’s use advanced systems that facilitate more carefully coordinated patient care and make sure providers have the most up-to-date information about their patients’ health.
The goal of the ACO is to support our providers in communicating better with each other in order to avoid ordering duplicate tests or screenings. Working together, our providers and our Population Health Nurse can do more to follow our patient’s health and to make sure they get the best possible care.
Star Valley Medical Center chose to participate in an ACO because we are committed to providing patients with a better care experience.
