Q: Do I need a referral from my primary care physician?
A: While it largely depends on the type of medical care you need, if you have a referral doctor, we will communicate closely with them to maintain continuity of care. Doctor referrals are never required for life-threatening, emergency medical services.
Q: What do I need to bring to my first appointment?
A: If you are a new patient, or it has been more than a year since your last appointment with SVMC, please download, print and complete our new patient forms. Also, please remember to bring your photo ID, insurance card, and a list of any medications you’re taking.
Q: What are your regular office hours?
A: Our doctor’s offices are open 8:00 a.m. to 5:00 p.m. Monday through Friday, and closed on all federal holidays. Our urgent care facilities are open Monday through Saturday from 7:00 a.m. to 7:00 p.m. in Afton and Monday through Sunday from 7:00 a.m. to 7:00 p.m. in Alpine. Our emergency room is open 24 hours a day, 7 days a week.

A: Should you need to cancel or reschedule an appointment, please call 307-885-5800 at least 24 hours prior to your scheduled appointment time to avoid cancellation fees.
Q: Which insurance policies do you accept?
A: We accept most major insurance policies. To avoid unexpected fees, please call 307-885-5995 to confirm we accept your particular insurance in advance of your appointment.

Q: I got hurt while on vacation; can I go to your Urgent Care?
A: Whether you’re a resident of Star Valley, or just visiting, you can visit any one of our service providers by just walking in without an appointment. We treat a variety of urgent care conditions including coughs and colds, snake and dog bites, hunting and fishing accidents, falls from horses and motorized vehicles, and run-ins with wild animals like elk, deer and moose.
